Advance Care Planning

Advance Care Planning

Every day we plan for the smallest things. We plan what to wear, we plan what’s or dinner, we plan what TV show we want to watch at night. However, we often forget to plan for the big things in life - like our own health care. Advance Care

Planning is a way to begin planning for those important things. Advance Care Planning is a constant process of thinking, discussing, and planning for a time when you cannot make your own medical decisions. It is really planning for the unexpected. As you go through Advance Care Planning conversations, it will help you say, in your own words, what is the right care at the right place in theright time.

At Sacred Heart Health System, the MY PLAN (Making Your Personal Life wishes: Act Now) campaign is aiming to create exposure and awareness to Advance Care Planning.

Join us on the third Wednesday of any month from 10 a.m. to 11 a.m. in the Cancer Center Conference Room B to learn about the importance of Advance Care Planning, 5 keys steps, tools so start Advance Care Planning and Florida requirements for Advance Directives. To register, contact Kerri Riggs at 850-416-7705. Visit the Advance Care Planning site for more info.
