Emergency Department Patient Access and Referral Coordination
A hospital emergency department is not always the best place to be treated for minor illnesses and injuries. If you come to the emergency department at Sacred Heart Hospital on the Gulf and you do not have a regular physician, then a Patient Access Advocate will help you find a primary care physician who can take care of your primary care needs and minor problems in the future. If you need follow-up care after the emergency department visit, then the Patient Access Advocate will help you schedule an appointment with a primary care physician or dentist.
The Patient Access Advocate can also help you with other needs. For example, if you need food or clothing assistance, help applying for the hospital’s financial assistance program, or a referral to a community social service agency, the MyGulfCare Patient Access Advocate can provide you with information on who to contact and can also help schedule an appointment for you.
If you have a chronic or urgent condition, are on Medicaid or are uninsured, you know how difficult it can be to find a specialist. MyGulfCare’s Referral Coordinator will work diligently to find you a specialist who will treat you regardless of your insurance status.
