Medical Information Print-Out
In a medical emergency, it can be hard to remember all the information you need - things such as allergies, insurance numbers, etc. So it's a good idea to have all the information in one place.
Simply click on the link for the form. Then, either fill it out online and print it, or print it out and fill it in by hand. Either way, please make enough copies. Any information you enter will NOT be saved once you close the window. This is to protect your privacy. When you're done, simply close the form window, and continue using our site.
Here are the specific instructions to follow for using the print-out:
Fill in the form, and then print it out, making as many copies as you need. Then clear the form, and make additional forms for each family member. Alternatively, you can print out multiple copies of the form, and fill them in by hand for each family member. When printing, make sure you're printing the form, not this page you're reading now.
Whenever you need to update the form(s), you can come back here and print out new one(s).
Make sure you keep the form(s) updated and stored in a convenient, safe place. Then if you need this kind of information, for example, if you need to go to the hospital or doctor's office, it's readily accessible.
