Case Management Program
Assisting Those Living with HIV/AIDS
Sacred Heart Hospital in Pensacola is proud to work with Lutheran Services to provide the Ryan White Title II Case Management Program, serving those in our community living with AIDS and the HIV virus.
Our program is designed to provide comprehensive information and assistance to clients and their families in areas including outpatient medical care, dental care, mental health services, and medications. Through this program, clients and their families are also linked with local agencies that can assist with needs related to housing, transportation, drug and alcohol treatment, and food.
Overall, the objectives of Sacred Heart’s Ryan White Program are as follows:
- To increase the quality of life for people living with HIV/AIDS
- To provide access to available resources in the community
- To improve service coordination between client and area agencies
- To eliminate barriers to health care services
- To reduce the cost of care by coordinating services that keep clients with HIV/AIDS out of the hospital
- To provide client advocacy and crisis intervention services
The services provided through the Ryan White Program at Sacred Heart are available to anyone who is a resident of the state of Florida living without medical benefits such as Medicaid/Medicare and living without the financial resources to acquire private insurance or access to medical care on their own.
Sacred Heart Health System collaborates on this program with a number of organizations in the community including Children’s Medical Services, North Florida AIDS Consortium (NoFLAC), Lutheran Services Florida, Escambia Health Department, Community Information Network (CIN), Appetite for Life, Emerald Coast Area Services and Education (EASE), Okaloosa AIDS Support and Information Services (OASIS), and Faith In Action/Covenant Hospice.
The program is named after Ryan White, a teenager who was determined to educate the public about AIDS before he lost his life to the disease. Congress approved the Ryan White CARE Act in 1990.
