Preparing for Your Visit

To ensure that your visit to Sacred Heart Urgent Care goes as quickly and smoothly as possible, our team has prepared a simple checklist to help you prepare for your visit.

Please bring with you your:

• Current valid insurance card
• Current valid photo identification (i.e., Government-Issued ID Card or Driver’s License)
• Complete list of medications, including herbs and vitamins.  Your list should include the name of the medication, the dosage you are taking, and what condition you are taking the medication for.  You may bring your prescription bottles if you do not have a list of medications.
